Medical Social Workers are a vital cog in our home health team. They conduct a variety of services including psychosocial assessment of the patient and their environment; determine their immediate needs, wants, limitations and barriers affecting recovery.
They further identify the best community resources, financial support and assist with long term planning. Social workers help patients identify their strengths and work on them collaboratively to maximize their progress. At Ultimate One, Medical Social Workers work with the patient and their caregivers to provide services and keep patients in a safe and homely environment.
